I confirm no icon shows when (in my case) no connection was possible to a remote server.
- (Ofc I had no remote server link; in your test the remote server could reject the device for any number of reasons. Seems no connection, no icon)
The app exists (at 176MB) and is found in Settings > Apps.

This is an unusual list of both Trackers and Permissions. Did you try to test with ALL Advanced Privacy turned off?
An observation; the app description seems aimed at the employer with “benefits” seemingly to intrude on worker privacy (personal prejudice perhaps).
Maybe your employer can provide a troubleshooting link to the app devs.
