I recently got the same issue with the Belgian banking app ‘KBC Mobile’. It works fine for checking my balance etc. but the QR code scanner does not work anymore since a few weeks.
The QR reader has access to my camera as I see the ‘video’ feed, but it does not recognise the QR and seems to be in a faulty aspect ration (image appears to be stretched wider than it should be).
Using /e/ OS version of the Fairphone 3+ ordered via /e/. ( v0.17-q )

Any updates on why reading QR codes is generally not working?
For me BankID works nicely including scanning QR.
But Swish fail to scan QR. It displays the camera view live in its interface but never detect the code. Workaround is of course to enter the number manually.

I have also noticed some difficulties to detect QR codes on FP3 (not necessarily BankID or Swish). What turned out to be the problem was dark mode in the web browser (using Dark Reader extension). The used framework to detect the QR codes seems to have some problems with a dark background around the code in some cases.
